Education and career: The degree and the institution, then the present work if she works. One line each, written plainly — this is the row read straight after the name.
Height and date of birth: Feet and inches, and the date with the time and place if the family uses the horoscope rows.
Family: Parents' names and what they do, brothers and sisters and whether they are married, and the native place.
Horoscope rows: Rashi, nakshatra, gotra and manglik, if the family uses them. The community formats add the rows a particular set prints.
Contact person: Whoever the family wants the other side to call. The Contact Person row is separate from any personal number.
Photo: One clear, recent photograph. The form crops it; the floral cover designs place it in a circle on the front page.
